- W2009236585 abstract "Within a framework of Primary Health Care (PHC) and Health Promotion (HP), the focus of this paper is on integrating traditional and modern health care in the remote area of Bashkharka, Nepal. The relevance to integrated health care of factors such as geography, ethnicity, religion, national infrastructure and international development agencies are discussed. Issues concerning the relationship between the theory and practice of PHC and HP are raised. The provision of adequate health care services to all of Nepal's population has been hindered by inaccessible terrain, low literacy levels, poor communication channels, problems with transportation, inadequate financial and manpower resources, and some cultural and religious beliefs. An approach that integrates Western medicine with traditional medicine and ritual healing offers a feasible means of expanding access to health care in Nepal's remote regions. All of Nepal's hospitals and health posts are government-run. There is no insurance system, meaning that poor families who live within distance of a medical facility often cannot afford services. The primary health care system has been criticized for failing to consider villagers' values and perceived health needs and assumption that rural people indiscriminately follow traditional practices. Most of the rituals in traditional medicine center around child survival. While some of these practices are harmful (e.g., withdrawal of vegetables from the diet of breastfeeding mothers the ceremonial introduction of rice and salt into the infant's diet, and inappropriate fever reduction techniques), others are beneficial and should be introduced into the modern health care system. For example, the UK-based Save the Children Fund has launched a program in Bashkharka that builds upon existing resources by training traditional healers in the preparation of rehydration solution, simple dressings, infant nutrition, and scabies treatment. Similarly, the Department of Health in Bashkharka has trained traditional birth attendants. Field observations have found that trained healers can be quite effective and are more readily accepted and trusted by rural families than medical health workers." @default.
