• W2010864112 abstract "Introduction. Approximately 40 to 60 percent of HF patients have a normal or nearly normal LVEF. The physiopathological mechanisms underlying this condition remain uncertain. Hypothesis. Circulating concentrations of biomarkers of inflammation, extracellular matrix (ECM) turnover and neurohormonal activation in HF patients differ between patients with a reduced LVEF and patients with a preserved LVEF Methods: Prospective study of 58 patients with stable symptoms of HF, 29 with and a reduced LVEF (LVEF ≤35%) and 29 with a preserved LVEF (LVEF ≥50%). Concentrations of N-terminal B-type natriuretic peptide (NT-proBNP), C-reactive protein (CRP), procollagen type III amino-terminal peptide (PIIINP), matrix metalloproteinase (MMP)–2, MMP–9, and tissue inhibitor of matrix metalloproteinase (TIMP)–1 were measured by commercially available assays. Results: Patients in both populations had similar NYHA functional class (II/III: 59%/41% vs 62%/38%) and distance walked during a 6 minute walk test (346±71 vs. 312±115 meters). Patients with a reduced LVEF had higher concentrations of NT-proBNP compared to patients with a preserved LVEF (median: 1432 vs 269 ng/L; p<0.0001). No significant difference was apparent for the other biomarkers. The correlations between biomarkers significantly differed between the two populations. In patients with a reduced LVEF, NT-proBNP correlated with PIIINP (p<0.05), TIMP-1 (< 0.05) and MMP-2 (p=0.002); PIIINP correlated with TIMP–1 (p<0.05) and MMP-2 (p<0.0001); MMP-2 correlated with TIMP-1 (p<0.05). In patients with a preserved LVEF, the only significant correlation was MMP-2 and CRP (p=0.002). Conclusion. Our results confirm that HF patients with a reduced LVEF have higher concentrations of NT-proBNP compared to patients with a preserved LVEF despite presenting similar symptoms. Although we observed no significant difference in other biomarkers, the correlation between biomarkers differed significantly between these populations, with only a limited number of correlations in patients with HF and a preserved LVEF. This suggests that patients with a clinical diagnosis of HF and a preserved LVEF constitute a different, perhaps more heterogeneous population, which could explain the difficulties in identifying an effective treatment for this condition." @default.
