- W2010924932 abstract "Carcinoma of the esophagus is a highly virulent disease. Although surgery has generally been considered the standard therapy for local regionally confined esophageal cancer, the results remained poor with 5year survivals of 6–24% in Western countries [1]. In the results of a Japanese nationwide registration, the overall 5-year survival rate of resected patients was also reported as 24% which was published in 1989 [2]. Recent improvement of surgical result with radical nodes dissection has been reported as a control arm in Japanese randomized study comparing surgery alone with surgery followed by adjuvant chemotherapy [3]. The study showed significantly better survival with a 5-year survival of 45% than historical control of the prior study, though it did not show survival advantage from adjuvant chemotherapy. The authors felt that the survival difference might be caused by radical surgery. However, this statement is subject to interpretation due to differences between Japanese and Western patients, stage migration as a result of radical surgery, or patient selection in postoperative randomization which might exclude operative mortality and patients with poor condition after radical surgery. The currently available chemotherapieutic agents still have only modest activity for cancer of the esophagus. When used as single agent, response rates usually show only 15–30% with exceptional complete response (CR) and no survival advantage [4,5]. Various attempts at combining chemotherapy agents have been reported after the availability of cisplatin (CDDP).
