- W2011064556 abstract "Summary: A variety of surgical procedures have been described to manage patellofemoral malalignment and instability. An isolated proximal realignment was proposed by Insall et al. in 1976. They described a proximal soft-tissue procedure designed to be a dynamic realignment. The proximal realignment is not a mere capsular plication procedure but instead is a quadricepsplasty designed to redirect the pull of the quadriceps. Although the present surgical technique includes several modifications of Insall's original work, his general concept of the procedure has persisted. The procedure is meant to restore patellar congruence through a modified lateral retinacular release and realignment of the medial quadriceps. Careful attention to proper patellar tracking and extensor tension during the realignment is important. Several studies demonstrated successful results when this procedure is used to manage recurrent patellar subluxation or dislocation in properly selected patients. The procedure is not well suited for patients with advanced patellar chondrosis or arthrosis or for patients with patellar pain associated with chondromalacia. Proximal realignment is an excellent alternative in the surgical management of recurrent patellar subluxation or dislocation and may be used in both skeletally immature and adult patients." @default.
