Palliative care in the hospital setting is increasingly discussed, investigated, and implemented. However, when the patient is discharged into the postacute setting, including the home environment, the continuity of care often becomes disjointed and lacks clarity and structure. This presentation will present the business and clinical process of implementing and sustaining a home based palliative care program in the community, which includes a team of physicians, nurses, and social workers. The financial implications for initiating this type of community based service as well as the financial needs to sustain a program will be presented. The clinical benefit to the home bound patient will be reviewed, demonstrating the focus on the metrics to track the quality of life and the consequences of palliative care in the home to the future of health care. Productivity standards and staffing model examples will be discussed. This presentation will identify challenges and opportunities for sustaining a home based palliative care program." @default.
