- W2011570247 abstract "Some oocytes, 16-20 hours after sperm exposure, appear to be unfertilized yet go on to cleave and develop as embryos. These may represent viable embryos whose pronuclei were not visible due to accelerated or retarded progression through the fertilization process. This study was designed to compare the laboratory outcomes of embryos without visualized pronuclei at the time of fertilization check (Never Saw Pronuclei-NSPN) to normal bipronuclear (2PN) embryos. To determine if cycles in which NSPN embryos develop are compromised, 2PN embryos formed in cycles with NSPNs were compared with 2PNs from cycles without NSPN embryos. This was a retrospective data analysis from a single large private fertility center, collected from all IVF cycles from 1/1/2009 to 6/30/2013. An NSPN embryo was defined as an apparently unfertilized egg at fertilization check that subsequently was observed to have cleaved to a cellular stage by Day 3. Intracytoplasmic sperm injection (ICSI) cycles were analyzed separately from standard insemination cycles. Cycles employing both ICSI and insemination were excluded, as were cycles transferred on Days 2, 3 or 4. NSPN-developed blastocyst freezing and transfer rates were compared to these rates of 2PNs formed in cycles where there were no abnormally pronucleated zygotes as well as those from cycles along with NSPNs. Paired and two- sample t-tests were performed as appropriate. The blastocyst freeze rates and day 5 fresh transfer rates were significantly different between NSPNs and 2PNs. ICSI-derived NSPNs had lower freeze and transfer rates than insemination-derived NSPNs. There was no difference in transfer rates between 2PNs in cycles with and without NSPNs, and the freeze rates for 2PNs from the cycles with NSPNs were actually better. Significant results are shown below.Table 1Blastocyst Freeze and Transfer Rates of Embryos from 2PN and NSPN ZygotesInsem-2PN-only cyclesInsem-2PNs from cycles with NSPNsInsem- NSPNsICSI-2PN-only cyclesICSI-2PNS from cycles with NSPNsICSI-NSPNsBlastocyst freeze rate20.1b,k (N=723)24.5c,k (N=479)5.7a,b,c (N=481)19.2d,l (N=839)23.3e,l (N=341)2.4a,d,e (N=342)Day 5 transfer rate14.2g (N=573)12.5h (N=381)2.7f,g,h (N=384)13.7i (N=667)13.4j (N=276)0.4f,i,j (N=279)a,b,c,d,e,f,g,h,i,j,k,l: P<0.05. Embryos cleaving after no pronuclei are visualized at fertilization check have a very low blastulation rate, and are rarely transferred or frozen. Normal 2PN embryos, in cycles where NSPNs are also observed, have no variation in their development to quality blastocysts. Formation of NSPN zygotes does not portend poor development for the 2PN zygotes in the same cohort.
