- W2011737573 abstract "The advent of specific medical and surgical treatment for Parkinson’s disease (PD) in the 1960s and 1970s led many in the field to predict that disability and mortality of these patients would be substantially improved. 1 Mosso J.A. Rand R.W. Management of parkinson’s disease – combined therapy with levodopa and thalamotomy. West J Med. 1975; 122: 1-6 PubMed Google Scholar , 2 Hoehn M.M. Yahr M.D. Parkinsonism: onset, progression and mortality. Neurology. 1967; 17: 427-442 Crossref PubMed Google Scholar The seminal study of 802 patients by Hoehn and Yahr in 1967 reported that the standardised mortality rate of patients with PD pre-levodopa (L-dopa) was about three times that of the general population. 2 Hoehn M.M. Yahr M.D. Parkinsonism: onset, progression and mortality. Neurology. 1967; 17: 427-442 Crossref PubMed Google Scholar Although this had not been carefully studied prior to 1967, the ages of death in the Hoehn and Yahr study were the same as those reported for patients with paralysis agitans in the mid-19th century. These findings imply that there was no change in PD-related mortality between 1856 and 1964. In the early days of L-dopa therapy there appeared to be a reduction in mortality attributable to the new therapy, although subsequent analysis has disputed this interpretation. 3 Yahr M.D. Levodopa. Ann Intern Med. 1975; 83: 677-682 Crossref PubMed Scopus (21) Google Scholar Even with further therapeutic advances since the 1970s, including routine use of an entirely new class of medications (dopamine agonists) and new surgical procedures (deep brain stimulation), PD continues to impart a high rate of mortality. The prospective Sydney Multicentre Study of PD recently published 20 year data that has confirmed that mortality rates for patients being treated for PD into the 21st century remains at around 3 (standardised mortality rate). 4 Hely M.A. Reid W.G. Adena M.A. et al. The Sydney multicenter study of Parkinson’s disease: the inevitability of dementia at 20 years. Mov Disord. 2008; 23: 837-844 Crossref PubMed Scopus (1492) Google Scholar A diagnosis of PD appears, even with modern therapies, indelibly linked to an earlier death." @default.
