- W2011857776 abstract "To evaluate the safety and efficacy of Dakin's solution (DS) for irrigation of pancreatic fluid collections (PFC). This study includes 83 patients with pancreatitis complicated by PFC who underwent image-guided catheter drainage. Contrast-enhanced CT of the abdomen and pelvis was used for diagnosis of the PFC in all patients. Drainage indications were pain, sepsis and gastrointestinal or biliary obstruction. Single (17 patients) or multiple (62 patients) catheters were used for drainage. Follow up included irrigation with Dakin's solution 1-4 times per day. Parameters evaluated for were 1. DS safety: patient discomfort, evidence of malodorous discharge and clinically significant electrolyte abnormalities (Na+, Cl- and CO2); 2. DS efficacy: removal of necrotic debris and clearance of local infection; 3. Drainage success: PFC resolved with catheter drainage; 4. Recurrence: PFC reaccumulation after drainage catheter removal. 5. Drainage failure: need for surgery to complete evacuation of PFC or to treat recurrence and post procedural death. Of the 83 patients, 5 were lost to follow-up. A total of 78 patients are considered for DS safety and efficacy, drainage success, drainage failure and PFC recurrence. Post procedure electrolytes were obtained in 70/78 (90%). Dakin's was safe in 78/78 patients. The irrigant was well-tolerated, and there was no patient discomfort or malodorous discharge. In 70/70 patients, there were no significant electrolyte abnormalities. DS was effective in 78/78 patients. We observed a reduction in necrotic debris during the irrigation period and a clearance of local infection in all 78 patients. Percutaneous drainage alone achieved PFC resolution in 69/78 (88%) patients. PFC recurred in 12/78 (15%) patients. Drainage failure occurred in 9/78 (12%) of patients. Surgery was carried out in 2/78 (3%) patients (pancreatic necrosectomies and drainage). Demise occurred in 7/78 (9%) patients, 5 from multiorgan failure, 1 from pulmonary embolism and 1 from CNS herniation. In our series, Dakin's solution was safe for catheter irrigation of PFC. The efficacy of DS over other solutions was not evaluated in this study." @default.
