El objetivo de este estudio es estimar el riesgo a 10 años de experimentar un primer ictus en la población hipertensa española aplicando la escala de Framingham. Estudio transversal y multicéntrico realizado en centros de atención primaria de España que incluyó a pacientes hipertensos de 55 o más años (10 pacientes consecutivos por médico). Se midió la presión arterial (PA) conforme a las normas estandarizadas. Los riesgos se estimaron aplicando las tablas de Framingham para riesgo de ictus y para riesgo de enfermedad coronaria. Se incluyó a 16.129 pacientes, con una edad media de 67,7 años. El 57,1% eran mujeres; un 30,4%, diabéticos, y un 26,6% tenía antecedentes de enfermedad cardiovascular. El 20,3% presentaba hipertrofia ventricular izquierda en el electrocardiograma (frecuencia mayor en varones, diabéticos o pacientes con PA sistólica mayor o igual a 140 mmHg; p < 0,001). Cumplían objetivos de control de la PA el 30,1% de los no diabéticos y sólo el 5,9% de los diabéticos. El riesgo de presentar un primer ictus a los 10 años (desviación estándar) se estimó en un 19,9% (16,8), y el de enfermedad coronaria en un 19,2% (14,0). Mientras que en los varones el riesgo estimado de enfermedad coronaria fue mayor que el de ictus, en las mujeres este último fue mayor. El riesgo de ictus en la población hipertensa española es elevado y los principales factores que lo condicionan (edad, PA sistólica elevada, diabetes e hipertrofia del ventrículo izquierdo) tienden a aparecer agregados. El riesgo estimado de ictus en la mujer es mayor que el de enfermedad coronaria. El riesgo de ictus debe tenerse en cuenta en la evaluación y tratamiento del paciente hipertenso. Stroke is a major cause of morbidity and mortality in hypertensive population. The aim of this study is to estimate the 10-year risk of a first stroke in Spanish hypertensive population using the Framingham scale. Cross-sectional multicenter study in primary care enviroment, on hypertensive patients aged 55 or more (10 consecutive patients each physician). Blood pressure (BP) was measured according to standardized rules. Stroke and coronary risk were estimated using the Framingham scale. 16129 patients (mean age 67.7 years; 57.1% women; 30.4% with diabetes mellitus; 26.6% with previous history of cardiovascular disease) were analyzed. 20.3% showed electrocardiographic left ventricular hypertrophy (more prevalent in males, diabetics, with systolic BP ≥ 140 mmHg; p < 0.001). The goal of BP control was reached in 30.1% of non-diabetic patients and only in 5.9% of diabetics. The estimated 10-year risk of a first stroke (standard deviation) was estimated to be 19.9% (16.8), and the coronary risk 19.2% (14.0). While males had an estimated risk of coronary disease above the stroke risk, women had an estimated risk of a first stroke greater than the estimated risk of coronary disease. The estimated risk of stroke among Spanish hypertensive population is high, and the main risk factors (age, systolic BP, diabetes and left ventricular hypertrophy) tend to aggregate.
