- W2012073254 abstract "Background: CT coronary angiography (CTA) is increasingly used to detect coronary artery disease (CAD) but is limited in assessing its functional significance. Aim: To determine the incremental value of CT stress myocardial perfusion assessment when used in combination with CTA for determination of functionally-significant coronary artery stenoses using FFR as reference standard in patients with suspected CAD. Methods: Forty patients with chest pain underwent FFR and 320-row detector CT assessment including CTA and CTP. FFR was determined in all major vessels and assumed 0.5 in vessels with ≥90% stenosis and 0.95 in smooth arteries. FFR ≤0.8 indicated significant stenosis. CTA and CTP images were concomitantly assessed by consensus between two blinded observers. Results: Significant stenosis was present in 53% of patients and 33% of vessels. The presence of ≥50% stenosis on CTA had a per-vessel sensitivity of 0.97, specificity 0.78, positive and negative predictive values (PPV & NPV) of 0.68 and 0.97. The presence of both ≥50% stenosis on CTA and perfusion defect on CTP increased the specificity and PPV to 0.95 and 0.90, respectively with a sensitivity of 0.87 and NPV of 0.94. CTA had a per-patient sensitivity of 0.95, specificity of 0.68, PPV of 0.77 and NPV of 0.93. Combined CTA and CTP increased the specificity to 0.95 and PPV to 0.95, whilst the sensitivity and NPV remained preserved at 0.95 and 0.95, respectively. Conclusion: In patients with suspected CAD, combined CT coronary angiography and myocardial perfusion imaging can detect functionally significant coronary stenosis with excellent accuracy compared with fractional flow reserve." @default.
