- W2012144598 abstract "I share the concern expressed in recent papers [1–5] regarding deaths related to accidents involving intrathecal and epidural drug administration, but I am equally concerned at the growing clamour for the development of an alternative connection system. Bickford-Smith [5], while seeming broadly in favour, does at least indicate some of the problems associated with this approach, and I would like to emphasise these. Clinicians of my generation may just remember when there were two separate needle/syringe systems available, and the problems that could result, especially in the emergency situation. The general use of connections with a ‘Luer’ taper was a major advance. Any change would be extremely expensive and it is not without relevance to point out that the letter from Cyna and Simmons [6] appeared in the same issue of this journal as an editorial [7] addressing the problem of withdrawal of anaesthetic drugs solely because they were not profitable enough for the manufacturers. We do not want the same thing to happen with equipment. I agree that it is important to try and ‘engineer’ systems to minimise the risk of misadministration and would certainly favour a simple colour coding system for connections using red for arterial, blue for venous and yellow for regional block equipment because everyone is familiar with this coding from their first lessons in anatomy. However, provision of an entirely separate syringe, needle and catheter connection system would be both retrograde and, I suspect, prohibitively expensive." @default.
