This type of injury is unique in exposing patients first to severe pain, delirium, and the threat of annihilation and later to prolonged convalescence and disfigurement. The clinical picture, consisting of adaptive mechanisms such as regression along with emotional reactions such as grief, is not qualitatively different from the reactions to other severe illnesses.~ Yet in its depth and duration it reflects the tremendous stress and potential for human suffering resulting from this type of injury. That most patients who have been exposed to this experience recover without serious psychological sequelae is moving testimony to the strength of the human adaptive potential. The observations upon which this report is based were made primarily on 11 adults burned over greater than 30% of their body surface. These patients, having a mean age of 37 years, included eight men and three women. The burns covered an average of 47% of the body surface. All patients were treated on a specialized twelve-bed treatment unit for burns at the University of Iowa Hospitals where a larger study of the psychological reactions to burns is in progress. Three patients died. Those surviving remained on the treatment unit an average of eight weeks. A description of the psychological reaction to severe burns will be presented and followed by suggestions for the management of its more important aspects. A variety of factors, some more specific than others, contributed to the occurrence of the burns. Not all appeared to be accidental. Neurological disorders such as epilepsy and mental retardation occurred among the patients as did psychiatric disorders manifested by alcoholism and suicidal behavior. Certain" @default.
