- W2012302004 abstract "Despite different techniques, equipment, and statistical criteria, neuroimaging studies have been considerably consistent in the identification of brain regions involved in various pain related processes. Functional neuroimaging studies regularly identify activation differences in a stable set of brain regions between pain and control populations. Recent structural studies have reported similar gray matter abnormalities among multiple chronic pain populations. Yet, the findings from structural and functional imaging studies are not completely unified. While they may agree on certain structures (e.g., ACC), they may differ for others (e.g., thalamus, PAG). These parallel lines of research suggest that chronic pain may be associated with functional and structural plasticity (i.e., changes in brain morphology). Unfortunately, the relationship between chronic pain and brain morphology and functional activity is poorly understood. To address this need, we used Volume-Based Morphology (VBM) to compare the gray matter volumes of ROIs (regions of interest) previously associated with temporal summation of second pain using fMRI, between fibromyalgia patients and normal control subjects. Initially, we segmented the gray and white matter of each 3D-anatomical scan. This resulted in new 3D datasets representing only the gray or white matter for each individual. Subject specific gray matter volumes were determined by counting the number of non-zero gray matter voxels for each ROI. Independent Samples t-tests were used to analyze the resulting dataset. Preliminary results indicate that three ROIs have significant group related gray matter differences (left mid insula, left dorsal anterior cingulate cortex, and left rostral cingulate cortex), and in all three instances, fibromyalgia patients had significantly less gray matter than control subjects. Together, these structural and functional imaging results suggest a functional plasticity of the brain circuitry that is consistent with theories of central sensitization, and may be a key factor in the development and/or maintenance of chronic pain.
