• W2012306219 abstract "Introduction. – L’histiocytose X, ou granulomatose à cellules de Langerhans, est une maladie rare caractérisée par une accumulation de phagocytes mononucléés présentant des marqueurs des cellules de Langerhans épidermiques normales. Exégèse. – Nous rapportons l’observation d’une femme de 44 ans présentant une histiocytose langerhansienne hypothalamohypophysaire avec des manifestations cutanéomuqueuses génitales et buccales. Après divers échecs thérapeutiques, un traitement par thalidomide est instauré. Les lésions buccales et vaginales régressent alors rapidement. En raison d’une récidive 6 mois après l’arrêt du traitement, une nouvelle cure par thalidomide est entreprise, avec succès. Confirmé sur l'IRM, il est observé, parallèlement à la bonne évolution clinique et à la bonne tolérance, une régression de l’infiltrat granulomateux hypothalamohypophysaire. Conclusion. – Le thalidomide présente un intérêt nouveau dans le traitement des histiocytoses langerhansienne à expression cutanéomuqueuse, mais aussi dans les formes hypothalamo-hypophysaires permettant ainsi une stabilisation des lésions endocriniennes. Introduction. – Histiocytosis X or Langerhans cell granuloma is a rare disease characterised by the proliferation of phagocytes which have normal cell markers. Exegesis. – We report the case of a 44-year old woman with hypothalamo-hypophyseal Langerhans cell histiocytosis with genital and oral cutaneo-mucous involvements. After several non-active treatments, thalidomide began. Oral and genital lesions healed quickly. Recurrence of symptoms was observed six months after the treatment was stopped and a complete remission was obtained when thalidomide was reintroduced. The regression of hypothalam-hypophyseal granuloma infiltration was confirmed by MRI and the treatment was well tolerated by the patient. Conclusion. – Cutaneo-mucous Langerhans cell disease as well as hypothalamo-hypophyseal histicytosis may both benefit from a treatment with thalidomide." @default.
