- W2012316156 abstract "328 Historically, upper gastrointestinal complications, in particular peptic ulceration, were the cause of significant morbidity and mortality to transplant recipients. The aim of this study was to determine the prevalence and nature of foregut disease in the renal transplant population during an era of modern immunosuppression and anti-ulcer prophylaxis. Information was prospectively entered into a computer database over a 3-year period during which 236 patients underwent renal transplantation. For all patients with new upper gastrointestinal symptoms, data was collected with regards the presentation, investigation and the treatment received. Twenty patients (8.5%) developed symptomatic foregut disease. All patients had received anti-ulcer prophylaxis consisting of histamine receptor antagonists (HA, n=118) or proton pump inhibitors (PPI, n=118). Symptoms were reported in 11 (9%) receiving HA's and 9 (7.6%) of PPI treated patients. All individuals complained of epigastric pain and in addition 5 reported reflux and 2 had dysphagia. The findings of upper gastrointestinal endoscopy consisted of: Helicobacter pylori (HP) negative gastritis (n=6); HP positive gastritis (n=1); Gastro-oesophageal reflux disease (n=4); Cytomegalovirus (CMV) gastritis (n=5); Candidal oesophagitis (n=3); CMV oesophagitis (n=1). Patients with Candida and CMV were treated with appropriate antimicrobial therapy and the individual with HP +ve gastritis received irradication therapy. The 10 patients with non-infective causes were equally divided between HA and PPI groups, and were prescribed long-term PPI's. All patients are now asymptomatic and no patient required surgical intervention. The spectrum of gastrointestinal disease in the transplant population differs markedly to the general population with a high prevalence of opportunistic infection. The low prevalence of HP-related disease may be due to our own irradication therapy of HA/PPI and co-trimoxazole, which is used as prophylaxis against Pneumocystis infection." @default.
