- W2012532641 abstract "Objective Stenosis and calcification of cerebral arteries usually preceded stroke. Arterial stiffness predicts stroke, possibly because of its association with cerebral vascular lesion. Therefore, we assessed the association of various arterial stiffness indexes with silent intracranial arterial stenosis and calcification in hypertensive patients. Design and methods From the Putuo community in Shanghai, 298 untreated hypertensive patients (145 men, mean age 60.4±10.1 years) without a history of stroke were recruited. For each patient, computerized tomographic intracranial angiography (CTA) was performed, and the carotid–femoral pulse wave velocity (PWV) was determined by the Complior system. From the 24-hour ambulatory blood pressures, we computed the ambulatory arterial stiffness index (AASI) and the 24-hour pulse pressure (PP). Results Intracranial arterial stenosis and calcification were detected in 42 (14.1%) and 98 (32.9%) patients, respectively. After adjustment for confounders, PWV (odds ratio [OR] for 1 m/s increase: 1.25; 95% CI: 1.06 to 1.47; P=0.008) was independently associated with intracranial arterial calcification. Both 24-hour PP (OR for 1 mm Hg increase: 1.06, P<0.001) and office PP (OR for 1 mm Hg increase: 1.04, P<0.001) were also independently associated with intracranial arterial calcification. PWV and 24-hour PP significantly (P<0.001) associated with arterial stenosis. However, these associations became insignificant after additionally adjustment for mean arterial pressure. No significant associations of AASI with cerebral lesions were detected. Conclusions Increased arterial stiffness was independently associated with the risk of intracranial arterial stenosis and especially of arterial calcification. PWV was superior to other arterial stiffness measures in indicating the risk of intracranial arterial lesions." @default.
