Morrison and Meier have edited a significant new contribution to the field that helps clarify this debate by describing the intersection of both fields. Health care professionals in pain and palliative care have a lot to learn from this new book. In Geriatric Palliative Care, the editors have worked to unify concepts embedded in the practice of geriatric and palliative medicine, providing an evidence-based resource which explores common clinical issues facing the sick elderly, symptom management strategies shown to be successful, and guidelines which foster effective and informed communication. Geriatric Palliative Care is a 430-page textbook with 28 chapters contributed by leading authorities in the fields of geriatrics and palliative care. The book has the usual elegant look and feel of a work from Oxford University Press. The book is divided into five parts: an overview of the social and cultural context of aging as it relates to health care, disease-specific aspects of palliative care for the most common ailments affecting the elderly population, symptom management, communication, and systems of care for the chronically ill with palliative care needs. A significant feature is the characterization of the ‘frail elderly syndrome,’ which is a clear diagnostic construct in geriatrics, but not well known outside the field. The implications for palliative care are obvious. This illustrates the pattern of the book, in which the cardinal features of geriatrics are described, the intersection between geriatrics and palliative care is highlighted, and the role of palliative care in providing comprehensive care to this population is illustrated. The impact of the book is somewhat diminished by the variability among chapters. This is a typical shortcoming of multi-authored works. However, for the person who wants to read from cover to cover, we found the variability a bit startling. Some authors pursued a more philosophical discursive approach while others pursued a distinctly evidence-based approach. Nevertheless, all chapters were easy to read and engaging enough to hold the reader's attention. We found much of the content readily applicable to a broad spectrum of patients with progressive illness. The work contains relevant and timely information for all health care providers who work with the aging sick and their families, be they the primary care physician, hospitalist, geriatrician, oncologist or palliative medicine specialist. This work takes another important step toward the integration of palliative care principles into mainstream practice, shifting care to a more patient-centered base which combines life-prolonging and palliative interventions, emphasizes quality of life, maximizes function, and promotes effective communication between health care providers, patients, and their families. In summary, this is an important addition to the library of pain and palliative care specialists.
