- W2013111889 abstract "Background And Aim: Autoimmune hepatitis (AIH) is having wide geographical variation in its occurrence and clinical presentation. It is presumed to be a rare etiology for chronic liver disease in India. We did this study to know occurrence, clinical, biochemical and histological profile of these patients presenting to us. Methods: In this study all patients diagnosed to have autoimmune hepatitis from January 2011 to June 2013 were retrospectively analyzed. Data regarding clinical presentation, biochemical marker, autoimmune profile etc were retrieved from hospital records. AIH was diagnosed using simplified criteria for diagnosis of autoimmune hepatitis. Results: We evaluated 2895 patients among which 39 (1.39%) were fulfilling the criteria for definite AIH according to simplified scoring system. Among 39 patients 29 (74.4%) were female. Mean age of presentation was 37.6 years. Median CTP score was 8. The most common presentation was jaundice (52%). Examination revealed hepatomegaly in 16, splenomegaly in 10 and pedal edema in 4. Mean value for Biochemical parameters were bilirubin 6.84 IU/dl, SGOT 281 IU/dl, SGPT 230 IU/dl, ALP 185 IU/dL, albumin 3.3 gm/dl, INR 1.38. All patients were having hypergammaglobinemia. Autoimmune markers were ANA 25 (54.1%), Anti SMA 9 (23.1%), anti SLA 11 (28.2%), Anti LKM 17 (43.6%). Associated autoimmune diseases were also recorded. Hypothyroidism in 8 (20.5%), autoimmune hemolytic anemia in 3 (7.7%), DM 4 (10.2%), hypoadrenalism 2 (5.1%), myasthenia gravis 1 (2.6%), polymyositis 1(2.6%) and SLE 1 (2.6%). Two patients were having HCV RNA positive and one was having HBV DNA positive. Histological finding was showing interface hepatitis and rosette formation in all patients. We were not able to document emperipolesis in any liver biopsy specimen. Conclusion: This study demonstrates that autoimmune hepatitis is uncommon in India. Clinical parameters were similar to western population. Most of the patients were having ANA positivity, which can be used as screening tool for AIH. Association with myasthenia gravis and polymyositis were not reported before." @default.
