- W2013211401 abstract "Recently, the importance of folate deficiency and hyperhomocysteinemia (HHcy) for cognitive dysfunction has drawn much attention. However, the relationship between the degree of folate deficiency or HHcy and severity of hippocampal atrophy is not clear, as well as the relationship between the degree of hippocampal atrophy and recovery of cognitive function by folate supplementation is not clear. Cognitive function, hippocampal atrophy assessed by MRI, total plasma homocysteine (Hcy), and serum other vitamins were examined in patients with dementia showing the folate deficiency. The effects of the folate supplementation were also evaluated. Among 616 patients who visited our Dementia Outpatients Clinic, 26 patients (4.2%) (16 men and 10 women) who showed folate deficiency were enrolled. Mean age was 79.0 + 7.5 years old. Neurological examination, Mini-Mental State Examination (MMSE), and Hasegawa Dementia Scale Revised (HDS-R) were performed. The degrees of hippocampal atrophy in MRI were evaluated by voxel-based specific regional analysis system developed for the study of Alzheimer's disease (VSRAD), which yields Z score. HDS-R, MMSE and the Hcy value after folate supplementation were also examined. The mean folate concentration was 2.99 ± 0.46 ng/mL (normal range 3.6 - 12.9), Hcy was 22.9 ± 10.9 nmol/mL (3.7 - 13.5), vitamin B12 was 425.7 ± 216.6 pg/mL (233-914), and vitamin B1 was 41.80 ± 21.6 ng/mL (20-50). Mean MMSE, and HDS-R scores before treatment were 19.64 + 5.2, and 16.3 ± 5.2, respectively. The mean Z scores, indicating hippocampal volume, was 1.99 ± 1.79. Mean MMSE, and HDS-R scores after folate supplement therapy were significantly (p < 0.01) increased to 21.33 + 4.9, and 19.1 ± 4.7, respectively. The mean Hcy value was significantly (p < 0.01) decreased to 10.47 + 2.40. There was no correlation between the degree of MMSE, or HDS-R improvement and the severity of hippocampal atrophy by folate supplementation. For patients showing cognitive dysfunction with folate deficiency, cognitive function improved by the folate supplementation regardless of the severity of hippocampal atrophy." @default.
