- W2013424738 abstract "Background The optimal management of metastatic colorectal cancer (mCRC) involves a multimodality approach. Complete resection of limited metastatic disease is a critical, potentially curative, intervention for a minority of patients. Data for resection rates and outcomes in routine clinical practice are limited. Methods We analysed records of patients enroled in the Australian prospective mCRC registry TRACC (Treatment of Recurrent and Advanced Colorectal Cancer). Data collection commenced in July 2009, with 14 sites now contributing. Treatment intent was recorded at initial patient review as curative, potentially curative, or palliative. Findings Of 1012 patients, 213 (21%) underwent metastasectomy, 179 (84%) with R0 resections. Liver (55.4%) and lung (20.2%) were the most commonly resected disease sites. For 26 (12.2%) patients, the initial treatment intent had been palliative. Patients who had metastases resected were younger (median age 63 years versus 70 years, p p p p p p = 0.046) and a single disease site (61.3% versus 54.5%; p = 0.029) than had patients in public hospitals, but the median age of private patients was older (70 years versus 67 years; p = 0.041). Overall survival for resected patients was equivalent in both groups (median undefined, hazard ratio 0.96, 95% confidence interval 0.45–2.06; p = 0.919). Interpretation In routine practice, a substantial proportion of patients with mCRC undergo resection of distant metastases, including some patients initially considered incurable. Significant variation between sites has been noted, which might relate to differences in patient population or a more aggressive treatment approach. Multivariate analyses and review of individual centre data are planned to explore reasons for potential underuse of this critical intervention." @default.
