- W2014105389 abstract "老人性肺炎を起こす人は大脳基底核に脳血管障害を有する。そのため，ここに存在する黒質線状体の働きが悪くなり，ドーパミンの合成が減少する。ドーパミンの低下は頸部知覚神経節で作られるサブスタンスPの合成の低下を伴う。サブスタンスPは迷走神経や舌咽神経の知覚枝を逆行性に咽頭や気管に分布し，嚥下反射と咳反射を正常に保つ役割をしている。サブスタンスPの低下により両反射が障害され，口腔内唾液を口腔内雑菌とともに不顕性誤嚥を起こす。毎日起こしていて，いつか調子の悪いときに肺炎に至る。不顕性誤嚥を防ぐには第一にドーパミンを補充してやればよい。アマンタジンはドーパミンの合成をうながす。アマンタジンを3年間投与したところ，非投与群に比べて，肺炎の発生を約5分の1に減少せしめた。次にサブスタンスPが減少しているのであるから，サブスタンスPを上昇させるとよいことになる。カプサイシンを少量口腔内に入れることによって，サブスタンスPは強力に放出され，嚥下反射は正常化した。ある程度辛い食物を食べることも不顕性誤嚥を予防するために必要と考えられた。ACE阻害剤はサブスタンスPの分解酵素も阻害するため，サブスタンスPは少ないながらも分解されずに残り，正常な濃度になる。ACE阻害剤を投与することにより，肺炎発生を2年間で3分の1に減少せしめた。口腔ケアをすることにより，不顕性誤嚥はしても口腔内雑菌が少なければ肺炎には至らないと考えられる。2年間口腔ケアをすることによって，肺炎発生を非口腔ケア群に比べて40%だけ減少せしめた。" @default.
