- W2014339718 abstract "Much has been written in literature regarding orbital complications of sinus disease, but recent literature on optic neuropathy secondary to sinusitis is scarce.l Optic neuritis is generally defined as an inflammatory neuropathy of the optic nerve usually associated with infectious, inflammatory, compressive, neoplastic, or vascular processes. Degeneration or demyelination of the optic nerve occurs and leads to vision loss. It has been reported that multiple sclerosis is the most common cause of optic neuritis, and that most patients with optic neuritis without infectious etiology will actually later develop multiple sclerosis.’ The occurrence of optic neuritis secondary to paranasal sinus disease is very rare. A definite etiology in up to 50% of patients can never be determined even after long term observation3 When optic neuritis is associated with clinical signs and symptoms of orbital inflammation, and evidence of sinus disease on radiologic examination, the decision for surgical sinus exploration and drainage is straightforward. On the other hand, the presentation of isolated optic neuritis with posterior sphenoethmoiditis can pose a diagnostic dilemma, and many clinicians discount this association entirely.’ Complete visual loss under these circumstances has been described;4 however, the pathophysiological explanation of such extension of inflammation and sinus infection to" @default.
