La voie finale commune en est l'ischémie/hypoxie cérébrale et la mort neuronale. Face á cette succession de phénomènes, plusieurs approches thérapeutiques ont été proposées. D'un côté, il existe celle qui vise á augmenter la pression de perfusion cérébrale pour améliorer la perfusion dans les zones périlésionnelles;x se fondant sur l'autorégulation, cette approche diminue le volume sanguin cérébral dans les territoires sains et réduit ainsi la pression intracrânienne. De l'autre, existe le concept qui vise á diminuer la pression capillaire cérébrale, c'est-á-dire la pression hydrostatique responsable de l'œdème cérébral en cas de lésion de la barrière hémato-encéphalique et/ou en l'absence d'autorégulation. Ces deux concepts, a priori inconciliables, peuvent se révéler complémentaires, tout particulièrement lorsqu'ils se succèdent á bon escient dans le temps. Cette revue vise á revoir les principes de base de chacun de ces concepts et leurs applications cliniques. Various cerebral aggressions, either primary or secondary, can lead to the development of raised intracranial pressure. The presence of an elevated intracranial pressure often results in cerebral ischaemia/hypoxia and, eventually, neuronal death. In face of this cascade of events, several therapeutic approaches have been suggested. Two management concepts for patients with raised intracranial pressure have retained the most attention in recent years: the first suggests a therapeutic increase in cerebral perfusion pressure with the objectives to improve perilesional collateral perfusion and decreased cerebral blood volume, and consequently intracranial pressure in areas where autoregulation is preserved. The second concept supports the diminution in perilesional capillary pressure with the aim of decreasing vasogenic oedema. Although these two concepts are antagonistic and cannot be used simultaneously, they are probably complementary in the sequence of therapeutic events of patients experiencing severe head injury.
