- W2014571037 abstract "With the recent epidemic of pediatric obesity, several reports have indicated that the incidence of type 2 diabetes among youth has increased. While it is hypothesized that decreases in cardiorespiratory fitness (VO2peak) and physical activity (PA) may be contributory to the rise in type 2 diabetes in youth, to date, very little empirical data examining these issues are available. Therefore, the PURPOSE of this investigation was to examine VO2peak and PA in a group of previously diagnosed youth with type 2 diabetes and compare these results to recently published normative data for fitness and guidelines for physical activity in youth. METHODS: Forty adolescents with type 2 diabetes (17 boys / 23 girls, age 15.5 ± 1.8 yrs, Tanner 4.7 ± 0.6, duration 2.5 ± 2.0 yrs, HbA1c 8.6 ± 2.5) were assessed for VO2peak with a progressive exercise test on a cycle ergometer and moderate to vigorous PA was estimated via the 7-day PA recall. Fitness data were compared to age and gender adjusted normative values using NHANES 1999-2002 by Pate et al. and reported PA levels were compared to recent evidenced-based guidelines for youth by Strong et. al. RESULTS: Collectively, youth with type 2 diabetes were markedly overweight with a mean BMI of 33.7 ± 1.5 kg/m2. After controlling for BMI, boys exhibited significantly higher fitness levels compared to girls (2.8 ± 1.1 vs. 1.9 ± 1.0 L/min, p<0.001) while reported PA was not significantly different between genders (0.6 ± 0.1 vs. 0.5 ± 0.1 hrs/day). None of the youth reached age and gender adjusted criteria for healthy fitness while ~93% of boys and 94% of girls scored below the 10th percentile for VO2peak. Similarly, only 18% of boys and 22% of girls reported performing the recommended 60 minutes or more of moderate to vigorous PA per day. CONCLUSION: These results suggest that youth with type 2 diabetes exhibit low levels of cardiorespiratory fitness and the majority do not participate in adequate amounts of habitual PA. Although causality can not be determined from these analyses, our data imply that reduced fitness and a sedentary lifestyle may be integral components of the pathophysiology of type 2 diabetes among youth. Longitudinal studies are warranted to determine whether changes in fitness and/or activity over time impact diabetes progression or improvement in youth.
