- W2014645229 abstract "It is difficult to measure the cognitive function of an illiterate person with, the current conventional neuropsychological tests. Dementia may be more prevalent in the illiterate and lower-educated elderly, because a low level of education is a risk factor for dementia.The aims of this investigation were to develop a neuropsychological test battery for illiterate as well as literate persons and to assess the validity and reliability of the battery for the diagnosis of dementia. Four neurologists, three psychiatrists and a psychologist developed the Literacy Independent Cognitive Assessment (LICA). It consisted of story and words recall tests, stick design and visual recognition tests, number stroop test, animal fluency, Corsi block test, calculation, visual semantics test, and naming. Stratified-clustered-random sampling was used, and the sample size was 626 normal elderly persons who were administered the LICA. 293 normal controls were matched with 148 dementia patients and were administered the LICA by age, sex and education. Test-retest data of the LICA was obtained from 57 subjects within a mean time interval of 32.1 days. All subtests of the LICA were applicable to the illiterate subjects (98%) except three subjects who were unable to read Arabic numerals. The LICA was correlated with age and education, however not with sex. The LICA scores correlated well with the MMSE (r = 0.82). In respect to a diagnosis of dementia in literate subjects, the LICA (cut-off point 191) had a sensitivity of 94.8% and specificity of 90.3%, and the area under the ROC curve (AUC) was 0.986 (S.E. = 0.005). With respect to a diagnosis of dementia in illiterate subjects, the LICA (cut-off point 155.6) had a sensitivity of 92.3% and specificity of 87.5%. The AUC was 0.979 (S.E. = 0.009). The LICA was found to have a high test-retest reliability (r = 0.91). The, newly designed LICA is a useful neuropsychological test battery for the diagnosis of dementia in illiterate as well as literate persons.
