- W2014687730 abstract "Les différentes études menées sur les répercussions du tabac sur la fertilité masculine depuis une quinzaine d’années mettent en évidence une diminution de la qualité du sperme chez les fumeurs. En effet, les composants de la fumée de cigarette passent la barrière hématotesticulaire et entraînent de ce fait une altération des paramètres spermiologiques et de la qualité du noyau des spermatozoïdes. Au-delà de cette diminution de la fertilité masculine, le tabac semble également avoir un impact sur la descendance des fumeurs : embryons de moindre qualité, risques accrus de développer un cancer dans la prime enfance. Les mécanismes physiopathologiques en cause ne sont pas encore clairement établis, mais il semble que le stress oxydatif généré par le tabac soit une des hypothèses les plus probables. Il en résulte essentiellement une fragmentation de l’ADN qui compromet les chances de grossesse. En plus du spermogramme, le clinicien dispose désormais de quelques examens complémentaires pour évaluer cette fragmentation de l’ADN des spermatozoïdes (TUNEL Assay, SCSA…), disponibles dans des laboratoires spécialisés. Il peut également proposer un traitement antioxydant pour essayer de diminuer cette fragmentation et ainsi augmenter les chances de grossesse. The different studies conducted over the last fifteen years on the consequences of cigarette smoking on male fertility have shown a decrease of sperm quality in smokers. In fact, the components of cigarette smoke pass through the blood-testis barrier and thus induce an alteration of sperm parameters and nucleus quality of the spermatozoa. Beyond this decrease of sperm quality, cigarette smoking also appears to have an impact on the smoker's offspring: lower embryo quality, increased risks to develop a childhood cancer. The pathophysiologic mechanisms are not yet clearly understood, but one of the most likely hypotheses is the production of an oxidative stress which is responsible for DNA fragmentation compromising the chances of pregnancy. In addition to the spermogram, further tests available in specialized laboratories can be prescribed to evaluate spermatozoal DNA fragmentation (TUNEL Assay, SCSA…). Antioxidant treatment can be administrated to reduce DNA fragmentation and increase the chances of pregnancy." @default.
