- W2014688560 abstract "Summary Background Stroke patients should be managed in a stroke unit incorporating a team that includes, at a minimum, nursing, medical, physiotherapy, occupational therapy and speech therapy staff with specialist training in stroke. In remote and rural areas, specialist medical input may not always be available. Western Isles Health Board covers a population of approximately 26,000 with an average of 36 stroke patients per year. There is no locally available medical stroke specialist. Innovation A weekly stroke multidisciplinary meeting in the Western Isles Stroke Unit led, over a videoconferencing link, by a remote stroke physician. This link provided the capability for the specialist to “meet” patients and carers and to remotely review the medical case records and brain imaging via the Picture Archiving Communication System (PACS). We report a two-year service intervention with monitoring of length of stay and a focus group to assess acceptability. No other significant rehabilitation service changes were made during the 4-year period. Evaluation During the two-year project, 65 patients were managed by the telerehabilitation service. Median length of stay fell from 29 days in 2010 and 2011 to 17 days in the two year post-intervention phase, P = 0.07. No technical failures occurred. Qualitative data from members of the team and from the carer involved in the focus group was universally positive. Benefits were felt to come from empowering the team to discharge plan at an earlier stage. A potential recurring annual cost saving of €61,000 was identified. Conclusions The Western Isles Stroke Telerehabilitation (Specialist Medical Consultation) Service is feasible and acceptable. Numbers in the project were small but the intervention may have a positive impact on length of hospital stay with associated cost savings. It is a model that could be relevant to other remote stroke services and to other specialty areas." @default.
