- W2014787682 abstract "Barrett's esophagus is the premalignant lesion for adenocarcinoma of the esophagus and the gastric cardia. Recognition of Barrett's esophagus requires an upper endoscopy showing a columnar lined distal esophagus and the presence of intestinal metaplasia on histological examination. Careful identification of endoscopic landmarks, specifically the gastroesophageal junction, is crucial for the recognition of Barrett's esophagus. To impact the rapidly rising incidence of esophageal adenocarcinoma, more cases of Barrett's esophagus will have to be detected by a screening program. With the greater use of unsedated, transoral or transnasal, small caliber upper endoscopy, high-risk populations, which include older Caucasian males with chronic reflux symptoms, could undergo screening for Barrett's esophagus in a cost-effective manner, avoiding the costs and risks associated with the use of conscious sedation. Future technological advances in endoscopic (zoom and chromoendoscopy, virtual biopsy techniques) and nonendoscopic (balloon cytology) devices would help in increasing the accuracy of detecting intestinal metaplasia as well as reducing costs." @default.
