- W2014861064 abstract "448 We have used effluent retrieved from our organ donors to supplement bank blood in the liver recipient operation. The aim of this study was to establish the safety and effectiveness of blood procured from liver donors for use in liver transplantation. Eighty-one orthotopic liver transplants were analysed, 55 receiving blood retrieved from their organ donor, whilst 26 did not. Slight alterations of technique were used in an attempt to maximise the amount of blood retrieved. It was noted that greater volumes of cells resulted if the superior vena cava could be emptied - by instituting exsanguination 1 - 2 mins prior to caval crossclamping, or by avoiding crossclamping if the thoracic organs were not used. Greater amounts resulted if effluent was passively siphoned into a 5 liter bag rather than drained into a 3L cell saver reservoir under low suction. The effluent was passed through a Hemonetics cell saver and the resultant washed corpuscles resuspended in saline, and, after crossmatching, infused into the recipient as necessary. A mean of 740 mls, and up to 1750 mls of suspended red cells was retrieved. A median of 300mls less bank blood was needed if donor blood was used. The cell suspension had a mean Hb of 139g/L and mean white cell count of 15.6 ×10^9/ml. The maximum Potassium was 25mmol/L, however mean was 5.6. Sodium (149), Magnesium (0.56), Insulin (134 mcmol.L) were within acceptable levels and no event related to transfusion was seen in these patients. Patients receiving donor cells appeared to have a significantly higher incidence of acute rejection (63% vs 45%). More steroid was given to the donor blood group (median 1250mg vs 250mg), and more patients received OKT3(16% vs 8%). CONCLUSION: Donor blood is simple to procure and process, and safe to administer. Our results have suggested an increase in rejection and further studies to confirm this trend in a larger group and to elucidate the mechanisms involved are indicated." @default.
