- W2014948944 abstract "To test whether structural changes in pelvic bone marrow (BM) occurring in response to pelvic radiotherapy are correlated with BM metabolic activity. This study includes 6 patients (5 cervical, 1 anal cancer) with stage I-III invasive carcinoma treated with concurrent chemoradiotherapy. External beam radiation doses were 45 Gy in 25 daily fractions; the patient with anal cancer received simultaneous integrated boost of 50 Gy in 25 fractions to gross tumor. Chemotherapy was concurrent weekly cisplatin for cervical cancer and concurrent 5-fluorouracil and mitomycin-C for anal cancer. All patients underwent CT simulation plus baseline whole body 18F-FDG-PET and quantitative fat fraction (FF) MRI using a developmental quantitative version of Iterative Decomposition of water and fat with Echo Asymmetry and Least-Squares Estimation (IDEAL). All patients underwent mid-treatment (week 3-4) MRI-IDEAL scans; 4 patients also underwent post-treatment (week 6-7) MRI-IDEAL scans. The pelvic bone region, delineated by simulation CT of each patient, was registered to a template with deformable image registration; the resulting deformation field was then used to remap the FF and PET SUVbw (body weighted standardized uptake value) distributions back to the deformed pelvic bones. This resulted in a high resolution (3x3x2.5 mm) FF/SUVbw matrix (each with dimensions: 6x 39451), with each row corresponding to concatenated FF/SUVbw value in each patient. Voxel-wise correlations between the baseline SUV and baseline-FF and FF changes were tested. The mean (SD) SUVbw, FF, mid-treatment FF change, and post-treatment FF change in pelvic BM were 0.9(0.2), 49.5% (13.8%), 15.5% (6.7%), and 27.9% (6.2%), respectively. In each patient, we observed significant positive voxel-wise correlations between baseline SUVbw and baseline FF, mid-treatment change in FF, and post-treatment change in FF. Spearman's rank correlation coefficients between voxel-wise average SUVbw and baseline FF, mid-treatment change in FF, and post-treatment change in FF were 0.27 (p < 0.001), 0.65 (p < 0.001), and 0.68 (p < 0.001), respectively. Structural changes in pelvic BM in response to radiation are increased in regions of high glucose metabolism. Sparing functionally active pelvic BM subregions may reduce BM injury, potentially limiting acute and chronic hematologic toxicity." @default.
