- W2014954404 abstract "To describe the new screening marker for detection of open spina bifida (OSB) at 11-13 weeks and to evaluate its diagnostic performance. We prospectively performed detailed examination of fetal anatomy for more than 1500 high-risk pregnancies and fetal brain anatomy was routinely evaluated. Posterior-caudal displacement of the mesencephalon and its deformation (flattening) against occipital bone (“crash sign”) in axial view of the brain was considered as a marker for open OSB. The confirmation of the diagnosis of OSB was performed by visualization of meningomyelocele and by demonstration of the ventricular system collapse (dried-up brain phenomenon) on neurosonography. OSB was diagnosed in 13 cases and “crash sign” was positive in 12 of them (92.3%). No case of OSB was missed at 11-13 weeks. OSB was an isolated anomaly in 3 from 12 cases with positive “crash sign”. In other 9 cases OSB was associated with different anomalies: trisomy 18 (4 cases), trisomy 13 (1 case), triploidy (1 case), amniotic band syndrome (1 case), omphalocele-exstrophy-imperforate anus-spinal defects (OEIS) complex (1 case) and situs ambiguous (1 case). Brain abnormalities: ventricular system collapse (“dried-up” brain) or ventriculomegaly were present in all the cases of OSB with positive “crash sign”. In one case of OSB “crash sign” was negative (7.7%). This fetus was affected by OEIS complex and its brain had apparently normal structure. Posterior-caudal displacement of the mesencephalon and its deformation against occipital bone (crash sign) in axial view of the brain can be used as a screening marker for OSB. This marker is positive in vast majority of the cases of OSB irrespectively of the etiology of spinal defect. In all the cases of suspicious findings in the brain the diagnosis must be confirmed by direct visualization of the spinal lesion. Prospective studies in low risk population need to confirm the value of “crash sign” for 11–13 weeks OSB screening." @default.
