- W2015014097 abstract "The pacemaker twiddler's syndrome (PTS) was first reported by Baliss in 1968.1 He described a “twiddler” who had rotated her pulse generator several times on its long axis, rolling the lead wire around the boot and retracting the endocardial electrode into the neck adjacent to the right phrenic nerve and brachial plexus. The patient suddenly began to experience “irregular epigastric jerking movements, occasional spasms of involuntary inspiration and regular involuntary supinating movements of the right forearm and hand.” Subsequently, several reports2–5 described variations of the PTS theme. Common to all have been pacemaker failure or a complication arising from the pacemaker that resulted from spontaneous, subconscious, inadvertent or deliberate rotation of the pulse generator by the patient. We describe a patient recently seen in our clinic to illustrate this infrequent, yet important and easily identifiable clinical problem." @default.
