- W2015347655 abstract "Objectives: The Immunobead Binding Test (IBT) is one of the preferred methods for the detection of ASA but it can be labor intensive. A simpler, shorter assay would be useful in the clinical setting. The objective of this study was to determine the validity of a rapid IBT when compared to the longer test. Design: Prospective comparative study in a tertiary care infertility center. Materials and Methods: From March 1999 to February 2000, 32 male partners of infertile couples gave informed consent to participate in the study. When clinically indicated, semen specimens were analyzed using the standard IBT as described in the World Health Organization manual. A rapid method as described by Evans ML, et al. [Fertil Steril 70(2):344–349, 1998] was also performed in parallel. The results from the rapid assay were compared to and validated relative to the standard assay. Results: Using a threshold value of ≥20% as a positive result, 6 of 32 samples (18.8%) tested positive by the standard assay compared to 5 samples (15.6%) analyzed by the rapid assay. At that threshold, the sensitivity of the rapid assay was 83%, the specificity was 100%, the positive predictive value (PPV) was 100% and the negative predictive value (NPV) was 96%. Using a rapid assay threshold of ≥15% as a positive result, the sensitivity, specificity, PPV and NPV of the rapid assay were all 100%. There was a significant correlation between the results of the rapid IBT and the standard IBT (r=0.733462; p<0.000002). The average time it took to perform the standard assay was 5 hours while the time it took to perform the rapid assay was 1 hour. The cost of each standard assay was estimated to be $146 versus $23 for the rapid assay. Conclusions: (1) A rapid IBT has an excellent validity as a screening test for the detection of ASA in semen and has a highly significant correlation with the standard IBT. (2) The rapid IBT is brief and cost effective. (3) The rapid IBT can be used as a screening test for ASA during the performance of routine semen analyses using a threshold value of ≥15% as a positive result." @default.
