- W2015489583 abstract "Aim: Emergency surgical treatment of occluded colorectal cancer is associated to high mortality and morbidity. Self-expanding metallic stents (SEMS) have shown their efficiency in a palliative treatment of colonic cancer. The aim of this prospective multicentric study was to assess the efficiency of the SEMS endoscopically inserted for the emergency treatment of left colonic occlusion. Patients and Methods: Between January 2001 and August 2005, 45 patients, median age 67 [36-90] hospitalized in emergency in 11 institutions for left malignant colonic occlusion were included in a study after giving their informed consent. Two kinds of non covered SEMS were inserted : Wallstent Enteral and Ultraflex Precision (Boston Scientific®). The SEMS constituted either a temporary treatment before an elective surgery or a final treatment for occlusion in the presence of non resecable metastasis and/or a terrain with a contraindication for surgery. The main criteria was the relief of the occlusion syndrome. The secondary criteria were the possibility to carry out surgery in a single time without colostomy, the delay for surgery, morbidity and mortality at D30 linked to the endoscopic treatment and/or surgical treatment, length of the hospitalization of operated patients. Results: SEMS were inserted in the 45 included patients, allowing the resolution of occlusion in 43/45 (95%), 2/45 remained occluded, 1 was operated in emergency, 1 was treated by endoscopic dilation. A planed surgery was carried out in 20 patients (44%) with a delay of 8.1 ± 3.4 days between the two procedures. In 19/20 of them, tumoral resection with a single time surgery without colostomy was performed. In 20 patients (44%), SEMS was a final treatment of the occlusion. For two patients, pre-surgery radiotherapy was carried out for rectal tumor. One death (2%) linked to cancer evolution occured 4 days after insertion of the SEMS, 9/45 patients (20%) presented with complications. Three presented with a severe complication directly linked to the SEMS; 1 colonic perforation, 1 anal pain, 1 hemorrhage. Five complications occurred in the 20 patients who underwent a single time surgery without colostomy: 1 anastomotic leakage, 1 anastomotic fistula, 1 pneumothorax, 1 cardiac failure, 1 hypokaliemia. One colectasia without organic occlusion appeared in a non operated patient. The length of the hospitalization was 15.9 ± 11.2 days after planned surgery. Conclusion: Insertion of SEMS is efficient for emergency treatment of left colonic malignant occlusion allowing for a future planned surgery or pre-operatory radiotherapy in 49% of the cases and making up a final palliative treatment in 44% of the cases." @default.
