This study determines the incidence and characteristics of this population, and explores whether or not culture driven treatment affects future rates of UTI complications. METHODS We identified women with uncomplicated recurrent UTIs using MarketScan data from 2003–2011. Recurrent UTIs were defined as 3 UTI visits (office or emergency room) associated with a prescription for antibiotics during a 12-month definitional period. Cases were excluded if they had a UTI in the year preceding the first UTI in the definitional period, or if they had any complicating factors (e.g. any abnormality of the urinary tract, a neurologic condition, pregnancy, diabetes, or currently taking immunosuppression). We determined incidence of uncomplicated recurrent UTIs and how many of these women had urine cultures, imaging, and cystoscopy associated with their condition. Propensity score analysis was performed to determine whether having a urine culture associated with >50% of UTIs affected rates of downstream resource utilization and complications (e.g. emergency room visits, hospitalizations, office visits, IV antibiotics, and pyelonephritis). RESULTS We identified 28,545 women with uncomplicated recurrent UTIs during our study period, accounting for an incidence of 102 per 100,000 women. Frequency of recurrent UTIs increased with age, and 61% of these women had at least one urine culture, 6.9% had imaging, and 2.8% had cystoscopy during the 12-month definitional period. Having a urine culture >50% of the time during the 12 month definitional period was associated with fewer UTI related hospitalizations (p=0.005) and less frequent use of IV antibiotics (p=0.002) in the subsequent year. CONCLUSIONS Uncomplicated recurrent urinary tract infections among community dwelling women are fairly common. Urine culture directed care in these women is associated with decreased utilization of high cost resources.
