- W2016029209 abstract "Uterine arteries hemodynamics is routinely assessed by means of Doppler qualitative analysis. Quantitative information on uterine blood flow during pregnancy would widely improve our knowledge on feto-placental pathophysiology. The aim of this study was to develop a new method to quantify uterine arteries blood flow in normal pregnancy on the basis of ultrasound and Doppler measurements and simulation results with a computational model of the uterine arterial circulation. i) Anatomy of the uterine artery and its branches was evaluated on ten patients at mid-gestation (19–22 weeks) using 3D-power-Doppler mode. ii) Diameter assessment and Doppler sampling of uterine arteries were performed at four different sites: prior to the uterine artery/external iliac vessels crossing over, 10–15 mm prior to uterine arteries bifurcation, corporal and cervical branches within 15 mm from their origin. Uterine arteries were evidenced using the power-Doppler mode; diameters (D) were measured on a perpendicular B-mode view of the vessels after removing power-Doppler, at the maximum magnification. Resistance indices and absolute velocities (V) were measured with a Doppler beam angle < 30°. iii) The above data were used to build various computational model of the uterine arteries. Local hemodynamics was simulated and coefficients (h) necessary to evaluate the uterine arteries blood flow were obtained. iv) Blood flows in the uterine artery and its branches were estimated by using the formula Q = h*V*3.14*D/2*D/2. Preliminary results on ten patients indicate that single uterine artery flow varied between 80–250 ml/min. Flow distribution between corporal and cervical branches varied with peculiar local arterial anatomies. Nevertheless, a prevalent volume stream through the corporal branch was observed in all cases (58–80% of total flow).
