Celui-ci a consulté pour des myalgies des membres inférieurs évoluant depuis 4 mois, résistantes aux opioïdes majeurs et invalidantes. Ce jeune patient suivait un traitement spécifique d’entretien d’une leucémie aiguë lymphoblastique en rémission, associant méthotrexate, purinéthol et sulfaméthoxazole/triméthoprime. La myopathie est apparue dans le mois qui a suivi l’introduction d’un traitement au long cours par oméprazole 20 mg/jour. Seul l’arrêt de l’oméprazole a entraîné la guérison de la myopathie en moins de 72 heures. Le test de réintroduction, réalisé en double aveugle contre placebo, a fait réapparaître les douleurs musculaires sous oméprazole dans les 48 heures. Le même test effectué 3 mois après la cessation du traitement antitumoral n’a pas eu de conséquences. À partir des rares données bibliographiques, nous discutons le rôle de l’oméprazole et de son interaction avec le méthotrexate. L’élimination rénale de ce dernier serait modifiée en raison du blocage par l’oméprazole du fonctionnement de la pompe H+/K+ ATP- ase dépendante. We present a case report of a painful drug-induced myopathy observed in a 13-year-old boy. The child presented invalidating myalgia of the lower legs which had progressed for four months and was refractory to opioids.This young patient had acute lymphoblastic leukemia and had achieved remission. He was taking a combination of methotrexate, purinethol and sulfamethoxazole/trimethoprime.The myopathy developed within the first month following introduction of long-term omeprazole (20mg/d). The myalgia completely regressed within 72h of withdrawal of omeprazole. A double-blind challenge was performed. Omeprazole iinduced muscle pain within 48hours.Based on this case and the sparse data in the literature, we discuss the role of omeprazole and its interaction with methotrexate. Renal elimination of methotrexate appears to be modified by omeprazole blockade of the ATPase-dependent H+/K+ pump." @default.
