- W2016095709 abstract "The migration of skilled workers from the developing to the developed world is of great concern in medicine. Prior research in India has identified some push-and-pull factors that influence this decision, which include a desire for higher income, access to more advanced medical technology, and better opportunities for one’s children. There has been no study of the factors specific to trainees in emergency medicine (EM). EM was recognized by the Medical Council of India (MCI) in 2009; there are only 37 seats that lead to the formal “MD” designation. We expect that in addition to the previously identified factors that influence migration, concerns about the status of the field will be important. By surveying both current trainees and graduates, we will identify the relative importance of both new and previously identified factors in influencing the migration decision. Data collection: A non-experimental mixed-methods approach is being used to answer this question. In the first phase, whose results are reported here, a purposive sample of practicing emergency physicians and trainees in India were invited to participate in an anonymous online survey using SurveyMonkey. For the second phase, participants will participate in focus group discussions to be held via Skype. Data analysis: Survey data were downloaded from SurveyMonkey and opened in Excel for initial inspection. Data were then analyzed in Stata 12.1, where descriptive statistics were calculated and logistic regression was used to determine which demographic and educational variables are most strongly associated with decision to migrate. Respondents (n=63) were divided between attendings (30%), and trainees (70%). The sample is geographically and linguistically diverse, with 11 native languages and 17 home states represented; 17.5% of respondents were female and 82.5% male. Of trainees, 58.1% plan to seek employment abroad, in the United Kingdom (32.6%), the U.S.A (11.6%), or the Middle East (11.6%). Of the variables considered, only having family abroad was found to be significantly associated with the decision to migrate (OR 4.3, P<.05). Sex, marital status, having children, having studied abroad, and having educational debt were not predictive of a desire to migrate. Better recognition of emergency medicine was the top reason for desiring to migrate (45.8%), followed by obtaining professional experience not available in India (16.7%), higher salary (16.7%), and access to more advanced technology (8.3%). When asked to choose the one factor that most influenced a decision to stay, family was most important (43.8%), followed by a desire to contribute to specialty development (37.5%) and a desire to give back to the country (18.8%). For 44% of trainees planning to migrate, MCI recognition of their training would make them consider returning. More than half of surveyed emergency medicine trainees plan to seek employment abroad. Other than having family abroad, demographic and educational variables considered were not predictive of the decision to migrate. In contrast, concerns about specialty status and national recognition are salient for many trainees in deciding where to practice.
