- W2016110157 abstract "To the Editor.— The single most useful agent in the treatment of angina pectoris is nitroglycerin, administered sublingually. We would like to report an unusual cause of treatment failure with sublingually administered nitroglycerin in a patient with coronary artery disease. Report of a Case.— A 69-year-old man had a ten-year history of rheumatoid arthritis and a four-year history of Sjogren syndrome. In October 1974, typical angina pectoris developed and the patient was hospitalized for evaluation. He was given nitroglycerin repeatedly but had no clinical response. During one episode of chest pain he was again given nitroglycerin, 0.4 mg sublingually, still with no relief. After 15 minutes, an oral examination revealed an undissolved nitroglycerin tablet under the patient's tongue. When water was given, the tablet rapidly dissolved, and the patient noted prompt relief of his chest pain. Future episodes of angina were easily controlled with nitroglycerin and water.
