However, the optimal interval for measurement and the threshold for intervention remains controversial. Previous studies are confounded by cerclage placement and inclusion of pregnancy reductions. These studies have suggested 2.5 cm as the optimal CL, but recommendations for the gestational age for evaluation have been varied. Our aim is to identify the most sensitive and clinically useful strategy for identifying pregnancies likely to deliver prior to 32 weeks. A retrospective cohort of 81 triplets undergoing serial CL ultrasound from 16 to 28 weeks gestational age were evaluated. Multifetal pregnancy reduction, cerclage placement, and those pregnancies delivered for maternal and fetal indications prior to 32 weeks were excluded. Using a receiver-operator characteristic curve it was determined that a CL of ≤3.0 cm at 24 to 26 weeks is the best predictor of preterm delivery less than 32 weeks. This was superior to using a CL of 2.5 cm as the sensitivity increased from 57% (CL 2.5 cm) to 74% (CL 3.0 cm) with minimal change in the specificity from 88% to 83%. The negative predictive value of 91% using a CL of 3.0 cm was reassuring at this gestational age. Cervical length at 24 to 26 weeks in triplets is useful in identifying those patients at increased risk of preterm delivery prior to 32 weeks.
