Robert Valeri, Shukri Khuri, and Gina Ragno. The supplement summarizes basic and clinical research that was conducted by Dr Valeri and colleagues at the Naval Blood Research Laboratory (NBRL) during the past four decades and previously published in 45 peer-reviewed scientific articles. Nearly half of these journal articles were published originally in TRANSFUSION. In the present supplement, Dr Valeri and colleagues reconstruct the history and scientific observations that support their thesis, namely, “that it is important to transfuse anemic thrombocytopenic patients with RBCs that have satisfactory viability and function to achieve a Hct level of 35 vol percent before transfusing viable and functional PLTs.”1 Today, in my hospital and I believe in most others, house officers are well aware that the optimal management of nonsurgical thrombocytopenic bleeding in an anemic patient requires correcting the Hct to 35 percent. This important principle of patient management and transfusion medicine was established relatively recently as the outcome of a series of clinical and laboratory studies, most of which were designed and conducted by Dr Valeri and colleagues at the NBRL. The present supplement guides TRANSFUSION's readers on a step-by-step walk-through of pertinent observations and provides a 210-article bibliography for those readers who seek further background and the opportunity to review the original data. In August 2006, TRANSFUSION published another supplement by Dr Valeri and Ms Ragno entitled “The Survival and Function of Baboon Red Blood Cells, Platelets and Plasma Proteins: A Review of the Experience from 1972 to 2002 at the Naval Blood Research Laboratory, Boston, Massachusetts.” That review was a compilation of previously published and unpublished data collected during studies performed on a unique colony of baboons that had been maintained for clinical research purposes for 30 years. The present supplement is a perspective by Dr Valeri and colleagues of how previously published studies were designed and conducted to collect the data required to unravel the complex interactions of temperature, RBCs, PLTs, and plasma-clotting proteins when anemic thrombocytopenic persons bleed. Dr Valeri is generous in acknowledging the contributions of other investigators. Although the subject of the supplement is Dr Valeri's 46 original articles, his analysis adds references to 165 additional articles by other investigators in the field. The present supplement is a tribute to Dr Valeri's 40 years of leadership at the NBRL and to the vision of his sponsors, including the Department of Defense, US Congress, US Navy, and the Navy's Bureau of Medicine and Surgery and Office of Naval Research. The mission of the NBRL was “to develop blood products and methods to treat military and civilian casualties.” The present supplement addresses only one of the many challenging medical and scientific issues shared by military and civilian casualties that were investigated at the NBRL. Readers of TRANSFUSION will have a treat when they read this supplement and follow the logic of how each experiment at the NBRL advanced the science of managing bleeding in anemic thrombocytopenic persons.
