- W2016391128 abstract "Overall patient management is the key to treating tendon lacerations. A rapid and accurate diagnosis based on characteristic alterations in limb function and location of the wound is followed by aggressive efforts to circumvent infection. Tenorrhaphy is recommended with three basic goals in mind: (1) minimize post tenorrhaphy gap, (2) minimize disruption of intrinsic vasculature, and (3) prevent adhesions. The three-loop pulley suture pattern has been shown to be most conducive to achieving these goals. A period of at least four weeks of external coaptation is indicated. If the DDFT has been transected a heel extension shoe will be required for an additional three months. When a tendon sheath is involved tenoscopy is recommended for irrigation. An intrathecal drain is often used to provide a means of postoperative irrigation and drainage. Irrigation with several liters of 0.1% povidone-iodine solution is recommended. Following intrasynovial tenorrhaphy it is recommended that the tendon sheath be left open or to create a window medially and laterally to the repair site so that the surrounding blood supply and cells can support healing. The overall prognosis for return to athletic function for flexor tendon lacerations is approximately 50% for flexor tendons and 70% for extensor lacerations." @default.
