- W2016511725 abstract "Almitrine is a triazine derivative which raises PaO2 and lowers PaCO2 in animals, but only when chemoreceptors are intact (Bull Eur Physiopathol Respir 1980; 16:195-208). In 8 normal men (27—48 yr), 1.5 mg/kg almitrine by mouth gave peak plasma levels of 260 SD 90 ng/ml at 2 hr, resting ventilation (VE) then being unchanged, but the ventilatory response to progressive isocapnic hypoxia at rest then rose from – 0.54 SD 0.5 to – 2.31 SD 1.74 L min−1 (%SAO2)−l, whereas the ventilatory response to CO2 (VE/Pet CO2) was unchanged at 3.21 SD 1.5 before and 3.33 SD 1.48 L min−1mm Hg−1 after almitrine. In 7 patients with “blue and bloated” COLD (4 M, 3 F; age 40–74 yr) FEV1 0.67 SD 0.141; PaO2 52 SD 8.5; PaCO2 46 SD 5 mm Hg 1.5 mg/kg of almitrine by mouth raised VE slightly from 12.9 SD 2.9 to 14.2 SD 3.8 L/min after 2 hr, whereas SaO2 rose from 86 SD 4.4% to 93.6 SD 3.6%, and remained at 92 SD 3.9% after 6 hr; PaO2 rising from 52 SD 8.5 to 62 SD 7 mm Hg at 2 hr, and PaCO2 then fell from 46.1 SD 5.3 to 42.2 SD 4.5 mm Hg, without a significant change in oxygen consumption. In 4 similar “blue bloaters” (6 other studies being in progress), 0.75 mg/kg of almitrine orally twice daily over 2 weeks raised the lowest SaO2 recorded during a night's sleep by 2–18%, in a single blind placebo controlled crossover study. Almitrine is an effective agent which improves gas exchange in COLD both by day and by night when given by mouth." @default.
