- W2016980720 abstract "In Reply: Dr. Sukhera has carefully outlined the dilemma facing many within the accreditation community. Despite the attempts of the Accreditation Council for Graduate Medical Education (ACGME) to improve health care as well as residents' well-being by ensuring that residents work in “humanistic” work environments, its ability to do so is limited by the blunt instrument of institutional accreditation requirements.1,2 Consequently, the ACGME's Office of Resident Services was established in an effort to address the conflict between the “needs of the public” and those of the residents.2 More specifically, that office was established “to help physician trainees in graduate medical education receive fair solutions to residency training-related concerns and formal complaints.”3 Therefore, a mechanism exists that allows residents to report a concern without filing a formal complaint, and per the ACGME, those concerns do not affect accreditation status.3 This office was opened in January 2009, and its impact is unknown. That being said, it is difficult to imagine how the ACGME can effect change for residents at a particular institution if the issue being raised does not address directly the ACGME's mission as an advocate for the public. It is due to this underlying contradiction that we agree with the assertion by Sukhera (in his letter) and Cohen4 that other organizations, including the Association of American Medical Colleges and other specialty groups, should assist residents in engaging in meaningful conversations with their training programs and disciplines. Perhaps it is through organized medicine, large physician groups with vocal and active resident constituents, and the necessary political support that the needed changes for resident advocacy could be realized. The option of using institutions of organized medicine rather than unions as resident advocates may be ideal, for it would prevent the inherent conflict that occurs between unions and “management.” It would allow issues to be addressed within the profession and maintain important relationships that would ensure an ideal training environment that, at its best, would foster lifelong mentor/mentee relationships.
