- W2017132370 abstract "Purpose The discovery of native LV recovery and criteria for explant of implantable LVAD system is very complex due to the heterogeneity of the population implanted and the low incidence of recovery. Over a period of 16 months five female LVAD patients were successfully explanted. There are several contributing factors in assessing for and discovering LV recovery that this population demonstrate, foremost that 80% of the series presented with hemolysis as an in initial presentation of LV recovery. Methods and Materials Following an initial occurrence of device thrombosis resulting in device explants in a series of women, a review of quality measures was undertaken to evaluate if any contributing factor could be identified. Patient profiles, echo results, speed management, anti-coagulation therapy and risk factor for device thrombosis were all assessed. Results The 5 patients were all female, presented in acute shock at time of HMII implant, and had a mean duration of 841 days on HMII support with a median of 950 days. Anti-coagulation therapy was customized to each patient; all were on Coumadin, 4 of the 5 were on aspirin. Four of the 5 patients presented for admission to hospital with hemolysis. TEE and right heart catheterization were used as clinically indicated to determine explantability. A full comparison of SV and EF pre and post explants was limited by quality of echo images. Conclusions LV recovery should be studied at regular intervals in the acute HMII population as well as be considered as a differential diagnosis in HMII patients presenting with hemolysis, especially in the younger female population.
