- W2017307621 abstract "<b>1207</b> <h3><b>Objectives</b></h3> Imaging modalities in epilepsy pre-surgical evaluation can significantly increase medical cost due to multiple procedures. The goal of our study is to validate the utility and analyze the cost effectiveness of each modality in order to maximize the benefit of pre-surgical assessment. <h3><b>Methods</b></h3> 16 pediatric patients (M:F= 5:11, mean age: 14) with intractable seizures which were evaluated by all imaging modalities including MRI, FDG PET, IC, IT, computerize subtraction analysis, EEG, phase 1 and phase 2 surgical grid placement and subsequent undergoing epilepsy surgery were selected retrospectively. Each patient’s imaging studies were randomized, grouped into 10 different combinations (table) and then blindly interpreted by two epilepsy imaging specialists using consensus agreement. The location of epileptogenic focus by region (lobe) and by hemisphere was recorded. Using successful surgical outcome as a reference, each combination of imaging modalities was analyzed. <h3><b>Results</b></h3> See Table. <h3><b>Conclusions</b></h3> PET is slightly better than IC-IT SPECT in performance overall. However, at a given cost, IC-IT SPECT is significantly more cost effective than PET. Therefore, PET or IC-IT SPECT should be considered as a first-line imaging" @default.
