- W2017820413 abstract "In reply: We appreciate Dr. Glick's comments about our article. As expected, we agree with some of his statements and disagree with others. To begin, we disagree with his contention that the potential spread of performances among students accepted to medical school is so small that it can not be detected by an examination. Dr. Glick is correct in stating that all applicants admitted to our school are considered capable and meet minimum standards. However, differences in the performances of medical students are detectable, as is evidenced by their performances on medical school evaluations (examinations) and the United States Medical Licensing Examination (USMLE), where some students do very well while others fail. In addition, we were not attempting to predict performance in only a grade-based manner. We were attempting to evaluate the ability to predict differences in physician—patient communication as assessed by patients' satisfaction. The increasing emphasis in the United States on the need to choose medical students who are effective communicators is indicated by the potential addition of testing with standardized patients to the USMLE Step 2 examination. We concede that the lack of correlation in our study raises many questions. Was our admission process adequate to detect differences that do exist? Was the objective structured clinical examination inadequate? Or, did differences in students' communication performances, present at the time of their admission, “wash out” after three years of medical school training? These possibilities are all acknowledged in our discussion. We completely agree with Dr. Glick that most medical education research would be much stronger with randomization of subjects. In fact, we are quite envious of the University of Newcastle's ability to carry out such a study. Nevertheless, quasi-experimental designs such as ours remain accepted research methods. It was in the hope of refining the method of selecting better communicators for admission to medical school that we completed our study." @default.
