- W2017962781 abstract "Unfavorable Results of Eyelid and Lacrimal Surgery: Prevention and Management. By Joseph A. Mauriello, Jr., M.D. Pp. 610. Butterworth Heinemann, Boston, Mass. 2000. Price $205. This textbook represents a significant contribution to the body of literature dedicated to oculoplastic surgery. While there are several textbooks on ophthalmic plastic and reconstructive surgery, there are few that focus on the unfavorable result. This complication-oriented approach allows the reader to predict, prevent, and manage problems as they arise. The format of the textbook is also unique, with each chapter concluding with expert commentary followed by editorial commentary. This provides a clear forum for discussion of controversial topics within the chapter as well as alternative opinions and ideas. The editor of the text is Dr. Joseph Mauriello, who has practiced oculoplastic surgery for more than 20 years. With a strong background in ophthalmic pathology from the Wills Eye Hospital, the editor has taken a problem-oriented approach and has enlisted more than 50 authors considered experts in the field. The impressive team of authors includes specialists from ophthalmology, plastic and reconstructive surgery, otolaryngology, dermatology, and pathology. Although preparation of the book took more than 2 years, the information is up to date and well referenced. The text is divided into three main sections: Part I: Aesthetic Eyelid, Eyebrow, and Midfacial Surgery; Part II: Reconstructive Eyelid and Facial Surgery; and Part III: Lacrimal Drainage Surgery. Part I has five chapters covering blepharoplasty, forehead lifts, and midface lifts. The strength of the content includes commentary from the experts and clear photographs before, during, and after surgery. Unfortunately, despite the title “unfavorable results,” several authors have taken the opportunity to include too many “favorable” aesthetic results. Nonetheless, the combined extensive experience of the authors is very comprehensive and difficult to find in other texts. Part II of the text has 11 chapters focusing on reconstructive surgery of congenital and acquired defects of the eyelids and periorbital region. The authors include recognized experts in the field and cover the global issues, including ptosis, ectropion, entropion, and lid retraction. The algorithm for ptosis correction is clearly described, as are options, including the Fasanella-Servat operation, frontalis suspension, and levator aponeurotic advancement. The diagrams in the reconstructive section of the text are superior to those in the aesthetic section and give the reader concise information on surgical technique. The addition of a chapter on repair of eyelid lacerations and Mohs defects adds worthwhile substance to the book. Dr. Mauriello has provided an excellent review of the ophthalmic pathology of neoplasms commonly encountered, including basal cell carcinoma, squamous cell carcinoma, malignant melanoma, and sebaceous carcinoma, as well as the indications for Mohs micrographic surgery. The concluding section, Part III, includes 14 chapters on the lacrimal drainage system. The anatomy of nasolacrimal duct and canaliculus is reviewed, including the bony and soft-tissue applied surgical anatomy as well as the intranasal anatomy, which is an often overlooked part of the lacrimal drainage system. The clinical evaluation of epiphora is clearly reviewed, as are the indications for surgery. Current surgical techniques, including minimally invasive endoscopic dacryocystorhinostomy and balloon catheter dacryoplasty, are included in the text. Although this section may be of less interest to plastic and reconstructive surgeons, a comprehensive text on oculoplastic surgery requires inclusion of orbital and lacrimal surgery. While the section on lacrimal surgery is complete, the editor has elected to omit orbital surgery, which is admittedly outside the scope of the text. The editor and the entire group of authors are to be congratulated on the creation of a new textbook in the specialty of oculoplastic surgery. Similar to the approach taken in the book entitled The Unfavorable Result in Plastic Surgery by Drs. Goldwyn and Cohen, this text teaches us through a problem-oriented manner how to avoid and manage the very complications that others have encountered before." @default.
