- W2018016626 abstract "We hypothesized that maternal genotype influences why some women with a short CL have a PTB whereas others deliver at term. Secondary analysis of a multicenter RCT of 17-alpha hydroxyprogesterone caproate for PTB prevention among nullips with an asymptomatic short mid-trimester CL (< 30 mm). Maternal samples underwent whole exome sequencing. Exomes were compared between women with PTB <34 wks (cases) and those with term deliveries >37 wks (controls) using the Variant Annotation, Analysis & Search Tool (VAAST), a probabilistic search tool for identifying disease-causing variants. We then generated a ‘PTB’ phenotype in the Phenotype Driven Variant Ontologic Re-ranking tool (PheVor) to re-prioritize and annotate genes identified by VAAST. Top genes identified by VAAST were classified into biologic pathways/gene ontology groups by the Protein Analysis Through Evolutionary Relationships (PANTHER) online tool. Gene distributions within these classifications were compared to an online referent population to identify over- and under- represented gene sets. 107 women were included; 10 (9%) delivered <34 wks. Baseline characteristics, including CL parameters, did not vary between groups. All samples met genotype quality filters. We allowed for locus heterogeneity and recessive inheritance in VAAST analyses. PheVor re-ranked the raw list of VAAST genes; selected variants ranked by Phevor are shown in the Table. COL14A1 was identified by both VAAST and PheVor as a possible candidate gene for PTB <34 weeks; Atlas 2 expression data shows COL14A1 to be highly expressed in the uterus corpus. VAAST identified 366 top differentially represented genes between cases and controls, but upon analysis in PANTHER, the distribution of these genes in known ontology functions did not vary from referent populations. Although not reaching formal genome wide significance levels, these data suggest that maternal genotype may influence pregnancy outcomes among women with a short CL." @default.
