- W2018128511 abstract "Cleft lip (CL) and cleft palate (CP) are among the more prevalent fetal malformations. They usually account for up to 13% of their total number. CP is associated to 70% of unilateral CL and 85% of bilateral CL. However, CP often escapes classical 2D-ultrasound detection. We speculated that 3D-ultrasound could provide a useful new tool because it allows better visualization of palate by multiplanar reconstruction and surface imaging in order to further define CP when CL is detected prenatally in 2D-ultrasound. The files of 18 fetuses diagnosed with CL prenatally between June 1997 and December 2003 and referred to our unit were retrospectively studied. A conventional 2D and a 3D ultrasound were performed between 13th and 36th weeks in these pregnancies using Combison or Voluson 530MT (Kretz) from 1997 to 2001 and Voluson730 (Kretz, GE) in 2001. They were carefully reviewed and compared to the anatomical description provides during repair surgery or after autopsy if the pregnancy was terminated. A satisfactory viewing of palatal structures was achieved in 17 cases/18 by 3D ultrasound. This figure has to be compared with only 8cases/18 where 2D-ultrasound allowed a clear visualization of the palate. A total of 13 CP associated to CL were confirmed in the newborns and fetuses autopsies. If we now compare the quality of US anatomical description of the malformation, it was surimposable in 15cases/17 of the 3D descriptions and in 7cases/8 of the 2D descriptions. A diagnosis of an associated CP was done in 5cases/13 by conventional 2D-ultrasound and in 13 cases/13 by 3D technology. There were no false positive with 2D—ultrasound and 2 cases of false positive with 3D-ultrasound. Detection of palate clefting, in cases where cleft lip was the first detected malformation, is particularly successful when using 3D-ultrasound technology." @default.
